Before you start
Please make sure you have your evidence ready before starting this form.
You will need:
- PDF bank statements for the last 2 full months for every bank account you hold
- Your latest Universal Credit statement or other benefit evidence as a PDF
- Any bills, invoices, quotes or other supporting evidence as PDFs
All evidence must be uploaded in PDF format. Please do not upload screenshots, photographs or images of documents.
For bank statements, you must provide statements for ALL accounts you hold, not just your main account. If we can see transfers to or from another account, we will need the statements for that account too.
If you live with a partner, we will need to see statements for ALL accounts they hold in their name too.
